SJH HospiceCare also provides care in local nursing homes.  Nursing home residents may choose to elect hospice care when they become eligible.  Also, patients in their own homes who have elected hospice can retain their hospice care if they choose to enter a nursing home.  Hospice care becomes a supplement to the existing care provided by the individual nursing home. The hospice team and the nursing home staff coordinate the care of the resident.  All of the services that are available to the hospice patient in their own home, are also available to the hospice resident in the nursing home.

 Hospice Care in an Assisted Living Facility

SJH HospiceCare has also provided care to individuals living in local assisted living facilities. Residents of an assisted living facility may elect hospice care when they are eligible. Also, patients in their own homes who have elected hospice can retain their hospice care if they choose to enter an assisted living facility. All of the services that are available to the hospice patient in their own home, are also available to the hospice resident in an assisted living facility.
